The Court will charge you a filing fee of $370.

Please complete this form to gather all the information we need to prepare your application. The information will form part of your affidavit – a written statement sworn or affirmed true and filed in Court – so it is important that it is accurate.
To complete this form, you’ll need the following information:
- Will
- Death certificate
- Information about the deceased's assets and liabilities
- Contact details of any person appointed as an executor
